Order forms are available on this website, in store from OASA, or, if receiving your supplies by post your package will include a blank order form.

You can place an order by completing an order form then send it in 4 ways:

1. Email the pdf or excel order as an attachment to orders@colostomysa.org.au
2. Take a photo of your completed order form, attach to a new email and send to orders@colostomysa.org.au
3. Post to 1 Keele Place Kidman Park SA 5025. Mail can take 7-10 days for it to arrive at the office
4. Fax: (08) 8355 1073 (Post office or library may be able to fax it for you)
5. Deliver in person.
